SDC — Selective Dorsal Cryoablation for Premature Ejaculation
What Is SDC?
If you feel like your body simply won’t cooperate — no matter how much control you try to maintain during intimacy — the cause is often not a lack of willpower. It may be something physiological.
SDC — Selective Dorsal Cryoablation — is a minimally invasive, in-office procedure that targets the dorsal nerves responsible for excessive penile sensitivity. These nerves are precisely identified, then treated with controlled cooling to reduce their reactivity.
The goal is not to eliminate sensation entirely. Normal sexual feeling is preserved — only the exaggerated, uncontrolled response is reduced. Performed under local anesthesia, the procedure takes approximately 15–20 minutes, and patients can leave the clinic the same day.
Penile Hypersensitivity and Premature Ejaculation
Premature ejaculation is one of the most common sexual health concerns among men. Many men live with it in silence for years — trying different approaches, feeling frustrated when nothing seems to work.
There’s an important reason for that: not all premature ejaculation has the same root cause.
The Main Causes of Premature Ejaculation
Penile hypersensitivity: The penis responds to stimulation far more quickly and intensely than average. In some men, this heightened nerve density is simply how they’re built. SDC directly addresses this mechanism.
Psychological factors: Anxiety, low self-confidence, relationship stress, or past experiences. These cases are best approached through sexual therapy or counseling.
Hormonal and neurological factors: An underlying systemic condition may be involved. A comprehensive evaluation is needed.
How This Condition Can Affect Daily Life
Sexual encounters ending far sooner than desired
Difficulty spending meaningful time with a partner, leading to loss of confidence
Growing performance anxiety during intimacy
Little to no benefit from sprays, creams, or medications
This is not a character flaw or a sign of weakness. Just as some people are more sensitive to sound or light, heightened nerve sensitivity is a physiological reality — and it can be medically addressed.
How Is SDC Performed?
SDC does not require hospitalization or general anesthesia. It is performed entirely in the clinic.
Step 1 — Specialist Evaluation
The source and degree of hypersensitivity are assessed first. Assoc. Prof. Dr. Akif Diri conducts a thorough examination and patient history to determine whether SDC is appropriate. The procedure is only recommended for suitable candidates.
Step 2 — Local Anesthesia
No special preparation is needed beforehand. Local anesthesia is applied to the area immediately before the procedure, achieving partial numbing before treatment begins.
Step 3 — Nerve Mapping
The dorsal nerve points responsible for excessive sensitivity are precisely identified. Only the targeted nerves are treated — surrounding tissue is not affected.
Step 4 — Cryoablation
The identified nerves are treated with a specialized applicator using controlled cooling. This reduces the nerves’ excessive signaling capacity. No pain or burning is felt during the procedure. Note: during nerve mapping, patients may experience mild tingling — this is normal and expected.
Step 5 — Completion
No incisions or sutures are required. After a brief rest period, patients can leave the clinic and drive themselves home.
The procedure takes approximately 20 minutes. Total time at the clinic, including preparation, is around 45–60 minutes.
Who Is a Good Candidate?
SDC may be a suitable option for men who:
Experience premature ejaculation due to penile hypersensitivity, confirmed by a specialist to be physical in nature
Have not achieved satisfactory results with topical sprays, creams, or oral medications
Prefer not to use — or cannot tolerate — ongoing pharmacological solutions
Are looking for a long-lasting option that does not require application before every sexual encounter
Want a meaningful result without undergoing surgery
Important: SDC is not appropriate for every case. When premature ejaculation stems from psychological, hormonal, or relational factors, other approaches will be more effective. Accurate diagnosis is essential to choosing the right treatment path.
Advantages
Pain-free: Local anesthesia ensures no pain, burning, or discomfort during the procedure.
Fast and convenient: Completed in approximately 20 minutes. Patients return home the same day.
Sensation is preserved: Normal sexual pleasure and feeling are maintained. Only excessive sensitivity is reduced.
No impact on erectile function: The mechanism behind erections and the nerve structures targeted by SDC are entirely separate. Erectile function is not affected.
Long-lasting results: The change in nerve signaling is durable — not temporary. No pre-encounter medication, spray, or cream is needed.
Not a surgery: No incisions, sutures, general anesthesia, or hospital stay required.
Recovery
No dedicated recovery period is necessary.
First 1–2 days: Sexual activity should be avoided. Mild sensitivity or swelling in the treated area may occur and will resolve on its own. The effects of the procedure may become noticeable quickly.
Weeks 2–4: The full effect develops gradually during this period as nerve signaling stabilizes.
Follow-up visit: A follow-up appointment is scheduled to evaluate results and monitor progress.
Daily activities: Patients can resume all normal routines on the day of the procedure.

Frequently Asked Questions
Will the procedure hurt?
No. Local anesthesia is applied beforehand, so no pain or burning is felt during treatment. However, because nerve stimulation is used during the mapping phase, you may notice mild tingling — this is entirely normal.
Will I lose sensation?
No. SDC does not eliminate sexual sensation. It reduces only the exaggerated hypersensitivity. Normal feeling and pleasure are preserved.
Could this affect my erections?
No. Erectile function and the nerve structures targeted by SDC are independent of each other. No negative effect on erections is expected.
When will I notice results?
Most patients begin to notice a gradual difference within 2–4 weeks as the nerve response stabilizes.
How long do results last?
The change in nerve signaling is durable. Individual outcomes may vary, which is why regular follow-up is recommended.
How many sessions are needed?
A single session is sufficient in most cases. If additional treatment is needed, your doctor will advise accordingly.
Is this procedure right for me?
That can only be determined through a specialist evaluation. Schedule a consultation with Assoc. Prof. Dr. Akif Diri to discuss your specific situation.
